
About GAPP
The Georgia Pediatric Program (GAPP) of the Georgia
Department of Community Health (DCH) serves eligible
children under 21 years of age who are medically fragile
and in need of medically necessary skilled nursing care
and/or medically necessary personal care support.
Eligible members should currently be receiving physician ordered services to be considered eligible for this program.

​Faith Home Care accepts patients via Georgia Pediatric Program (GAPP) offering skilled nursing and personal care services to medically fragile children under 21, at no cost to the family.

Become a paid caregiver through state Medicaid program!
If someone with a disability already receives Medicaid, their state may allow a family member or friend to become a paid caregiver.

Our Intake Specialist will schedule a visit at your home to complete an initial assessment and intake packet, which includes consent forms, and other necessary qualifying information.

Faith Home Care will Submit your information to GAPP.

Faith Home Care receives decision (approval may take up to 30 days)

Once Approved, Faith Home Care will contact you to arrange a meeting with a caregiver and finalize a care schedule, or to discuss the appeal process if needed.
